We’re excited to launch our new and improved claim form on Tuesday 8th April 2025. When you log in to the LeavePlus Portal to claim a Long Service Leave Benefit to fund your well-earned break, you will be presented with the option to use the existing claim form or try out the new version. Here’s what you can expect from the new claim form:

  • The new form is designed to streamline your application process using smarter features. We will continue to make improvements and add new features based on your experiences, and we can’t wait to get your feedback.
  • We’ve simplified the process with easy-to-follow steps and user- friendly instructions. Instead of one long form, we’ve now condensed each step into new sections with clear headings and guidance text for ease of use.
  • We have included new features such as pre-populating your information into the form, real-time checks, pop-up boxes, and tool-tips to make your application easier to complete.

+ YOUR FEEDBACK IS KEY

  • Please complete the post-claim survey which you will receive by email. We will continue to gather your feedback and make updates to improve the claim form.

+ USING THE FORM

  • Update your contact details in your profile page prior to submitting your claim.
  • On the Submit a Claim page you’ll notice a button to use the new claim form.
  • Answer all the questions in the form.
  • Keep an eye out for emails from us. We will reach out with updates on your claim or if we need more information.

+ FAQS & SUPPORT

  • Who can use the form?
    • All Worker members who are eligible to claim a Long Service Leave Benefit from LeavePlus can use the new form. Working Sub-Contractors are not eligible to use the new claim form at present.
  • I didn’t get to pick my gift when completing the form. Do I still get to choose a free gift?
    • Once your Long Service Leave Benefit has been paid, you will receive an email asking you to select your free gift.
  • Who can I contact if I have further questions or need support?
    • You can complete a ‘Contact Us’ form in the LeavePlus Portal or call us via the details on our Contact Us page.
