Change of Entity

When there is a change to your business structure and your organisation gets a new ABN or ACN, you will need to complete the Change Of Entity form.

We will need to make changes to your existing employer member profile to ensure that all the current and historical information regarding your business can be updated or carried over to a new entity profile. This may mean that your employer registration number with LeavePlus will change.

Why do you need to let us know when your business structure changes?

By letting us know when your details have changed, we can make sure your records are up to date and ensure that you remain compliant with your obligations to LeavePlus.

What Information Do You Need?

To complete the Change of Entity form ensure you have the following:

  • Current Business Details
  • LeavePlus Employer Member Number
  • Current ABN /ACN
  • Current Trading Name
  • New Business Details
  • New ABN/Trading Name
  • Contact Information
  • Business Address
